- Accident occurred late Friday or early Monday morning.
- Accident was not witnessed.
- Accident happened while performing duties not related to the position.
- Accident occurred just prior to a labor strike.
- First report conflicts with history in medical report.
- Details of accident are vague or inconsistent.
- Incident not promptly reported to supervisor.
- Accident occurred in an unauthorized area.
- Tips about conflicting claimant activities from coworkers.
- No physical basis for disability. All indications are that the claimant has fully recovered.
- Claimant is never home and cannot be contacted via phone.
- Claimant has a history of self-employment or might be able to continue to perform a trade for cash while "exhibiting" traits of a disability.
- Claimant or claimant's attorney makes demands for quick or excessive compensation.
- Injury coincides with downsizing, plant closings or layoffs.
- Leads suggest your claimant is active.
- Conflicting rehab/doctor reports: one says the claimant is injured; the other disagrees.
- Claimant receives mail at a post office box or through an attorney and will not divulge an actual residential address.
